About Us
Hospice Partners of Southern California is a non-profit, State licensed, Medicare and Medi-Cal certified hospice that provides a comprehensive and progressive hospice program, including hands-on hospice care. In addition to all the Medicare required services listed on the Services page Hospice Partners also provides many additional hospice related services such as community education and extended one-on-one bereavement counseling.
Hospice Partners is available 24 hours a day, 365 days a year.
The goals of Hospice Partners services are:
- Patient and family centered team approach
- Maximize patient/family autonomy in decision making
- Comfort, control and dignity
- Pain and symptom relief
- Education and support to the patient and family
- Physical, emotional and spiritual comfort
Hospice Partners is based in Santa Monica and primarily serves the Westside of Los Angeles, including Malibu, Beverly Hills, and the Beach/Airport communities. Hospice Partners of Southern California is an agency of Hospice Partners, Inc., a not for profit, tax exempt organization affiliated with Wilshire Health and Community Services, Inc.
